AMI 5.8: follow-up booklet and prescription form for patients with COPD and heart failure

ANFIIDE provides two tools for home follow-up of patients after hospitalisation for COPD exacerbation or heart failure decompensation: a follow-up booklet and a prescription form.
The nomenclature of nursing procedures (NGAP) provides for a clinical monitoring and prevention session rated AMI 5.8 for patients returning home after hospitalisation for exacerbation of chronic obstructive pulmonary disease (COPD) or decompensation of heart failure.
To facilitate this follow-up, ANFIIDE has designed two documents to download and print.
The follow-up booklet
It supports the patient at home throughout their follow-up.
